Title IIID Health Promotion

PROGRAM DESCRIPTION

The Older American Act (OAA) Title III D Disease Prevention and Health Promotion program seeks to enable older adults' individuals to maintain their well-being through locally developed community-based systems of services. It provides evidence-based disease prevention and health promotion services. Title III D supports services that assist older adults in preventing illness and managing chronic conditions. These programs help older adults learn techniques and strategies for the prevention and mitigation of the effects of chronic diseases including osteoporosis, hypertension, obesity, diabetes, and cardiovascular disease. Evidence-based health promotion services include programs focused on alcohol and substance abuse reduction, smoking cessation, weight loss and control, stress management, falls prevention, physical activity, and improved nutrition. Individuals participate in services in various settings including multipurpose senior centers, community centers, and older adult housing.

If you would like to offer one of our programs at your community or senior center, please contact us at info@innovativehealths.com to get started.
